Student-Athlete Medical Eligibility

All Lindenwood University student-athletes must have a complete medical file with the Athletic Training Department and be cleared through the Athletics Compliance Office before athletic participation is permitted. Intercollegiate athletic competition includes any official team practices, voluntary workouts with coaches or other athletes, any team structured activities, and any use of the university fitness centers.
 
All student-athletes must complete an on-campus physical.  A completed medical file will include the required documents below that will be submitted via the Blue Ocean Portal System (Vivature), as well as a copy of the student-athlete’s current health insurance information. The student-athlete will be required to complete the necessary medical information, including emergency contact and insurance carrier information, through the Blue Ocean Portal system (Vivature). All information is subject to review by the Lindenwood University Sports Medicine Team prior to any type of athletic participation.

Reporting Injuries and Illnesses
Once cleared for participation, student-athletes are required to report all injuries and illnesses to the athletic training staff. The athletic training staff will work with the rest of the Lindenwood University Sports Medicine Team to ensure the appropriate treatment and referral for all conditions. This information will be communicated to the student-athlete’s respective coaching staff on a regular basis. After sustaining an injury or illness, all decisions regarding the student-athlete’s participation status for conditioning sessions, practices, games, and other supervised team activities will be made by the athletic training staff under the advisement and supervision of the team physician.
