Physical Forms - New Incoming Student-Athletes

New student-athletes are required to complete their athletic physical prior to coming to campus with their primary care provider (PCP) at home. The physical should be completed in person (not a telehealth appointment).  A Primary Care Provider may be a physician, a physician’s assistant or a nurse practitioner.  A physical completed by any other health care provider will not be accepted. The physical packet that needs to be completed can be found here. There is also a medical history included in that packet that the student-athlete will have to fill out prior to the visit.
 
Starting this upcoming academic year, the NCAA and Lindenwood are requiring all new student-athletes to provide lab results that indicate their Sickle Cell Trait Status.  This information is required prior to participating in any Lindenwood athletic team activities – NO EXCEPTIONS.  Information on how to obtain this information is located in the attached packet.
 
New student-athletes will still be required create a Vivature account and complete all pending forms (except Returning Athlete Medical History (RETURNERS ONLY)) on Vivature before they return to campus and are allowed to participate. Here is more information on how you can create your account in Vivature. 
 
New Student-Athletes also must complete ImPACT testing before returning to campus. They will receive an email directly from ImPACT by July 1st with further instructions.  Student-athletes should double check their junk mail if they do not receive instruction in their inbox. This will need to be completed before students arrive on campus. Below is a checklist to help ensure that you complete all steps:
 
NEW MEDICAL STUDENT-ATHLETE CHECKLIST
  • Create Vivature account
  • Complete all pending forms on Vivature (except Returning Athlete Medical History)
  • Upload primary insurance to Vivature using the KONGiQ app
  • Within Vivature, fill out “Student details”, parent contacts, contact (emergency)
  • Complete ImPACT Testing
    • You will receive a separate email with baseline concussion testing which needs to be completed.
  • Print off forms for appointment with PCP that are attached to this email
    • Fill out medical & cardiac history
  • Get appointment with PCP for athletic physical
  • Obtain a lab results indicating Sickle Cell Trait status 
  • Bring completed athletic physical paperwork (medical history, cardiac history and physician exam) and a copy of lab results indicating Sickle Cell Trait status with you back to campus
