Physical Forms - Returning Student-Athletes

For the 2024-2025 school year, returning student-athletes will follow a similar procedure as in past years for their athletic physical. Please follow these instructions. You DO NOT need to get a physical with your primary care provider prior to coming back to campus.
 
Click here for instructions that you will need to update Vivature.  You need to log in and update your personal information, insurance and complete all pending forms prior to coming back to campus.
 
RETURNING STUDENT-ATHLETE MEDICAL CHECKLIST
  • Update personal information within Vivature
  • Complete all pending forms on Vivature
  • Upload primary insurance to Vivature using the KONGiQ app
 
Your coach will inform you of your medical check in with the athletic training staff to determine your final clearance for participation. If you completed your medical check in last Spring with the athletic training staff you do not need to do it again. If you have changed your insurance since then please give your athletic trainer an updated copy.
